Westview Meadows & The Gary Residence are currently seeking a Resident and Family Services Director to help residents achieve and maintain optimum social, mental, spiritual, and emotional well-being and will serve as a resource for residents and their families. The preferred candidate will be eager to support our mission to provide a high-quality residential setting for seniors which promotes independence, individuality, choice, and security while supporting and enhancing the quality of life for the population it serves.
Responsibilities:
- Coordinates services for patients to transition from facility to facility.
- Works with RN & Executive Director in collaboration to coordinate and approve authorized continuum of patient’s care services.
- Review clinical resource options based on patient assessment and advised care plan after transition from either hospital care or SNF Care.
- Discharge planning to the next stage of care.
- Coordinate admission and develop a safe discharge plan with each individual and establish follow up care.
- Coordinates any services required for the patient to transition to their apartment in our facilities safely using home health care therapy programs, pharmacy arrangements, social work, etc.
- Meet with residents to discuss end-of-life decisions.
- Acts as a clinical liaison to Primary Care Physicians, referral sources, care coordinators home health physicians, and other essential resource teams.
- Assist residents in establishing living wills, powers of attorney, health care proxies, etc.
- Work with the resident, family, and management when probationary residency is needed.
- Communicates thoroughly and documents thoroughly to coordinate patient transition smoothly and efficiently.
- Complete social assessments on all residents.
- Carry out other responsibilities as assigned by supervisor.
Qualifications:
- Licensed Clinical Social Worker (preferred).
- Degree in social services from an accredited college or university
- Experience with case management.
- Flexibility and creativity in dealing with the issues being faced by residents and families.
- Ability to exercise independent judgement within the organization’s mission and philosophy.
- Experience and enjoyment in working with older adults.
- Understand and conceptualize complex concepts, as well as ability to make appropriate decisions in stressful situations.
- High level communication skills, both verbal and written.
- Must be proficient in basic office equipment, including a personal computer, for the purpose of accomplishing and maintaining a high level of job performance.
- Must pass appropriate background checks and be fully vaccinated against COVID-19.
Pay: $70,000.00 – $75,000.00 per year
